During the Questionnaire you will be asked to supply digital images of logos and of your facility. If these are unavailable in a digital format check the 'Not available' check box. If your images and logo are available in printed form, they can be mailed directly to our office at 24 Birwood St, Frankston Vic 3199. All printed material will be returned upon completion of your 2 page website.
One of our consultants will contact you shortly about any materials that need to be sent.
Please be aware that this questionnaire covers all forms of Aged Care Facilities from Supported Residential Services (SRS) through to Nursing Homes. For those questions that do not relate to your Aged Care Facility please leave the question blank.
