During the Questionnaire you will be asked to supply digital images of logos and of your facility. If these are unavailable in a digital format check the 'Not available' check box. If your images and logo are available in printed form, they can be mailed directly to our office at 32a York St, Chelsea. All printed material will be returned upon completion of your 2 page website.
Please be aware that this questionnaire covers all forms of Aged Care Facilities from Supported Residential Services (SRS) through to Nursing Homes. For those questions that do not relate to your Aged Care Facility please check the 'Not Applicable' box for that question.
The traditional marketing plan we are pleased to say is having a positive response, with flyers and notices being posted in many facilities relating to the Aged Care sector.
